Care, Treatment, Training, Habilitation and support we provide
We provide in home mental health services, training in, or reinforcement of, functional skills and appropriate behavior related to the individual's health and safety, ADLs and use of community resources; assistance with medication management, and monitoring health, nutrition, and physical condition.
Development of ISP within 30 days to indicate the specific in home mental health support and services to be provided and the goals and objectives to be accomplished. A preliminary plan will be developed within 24 hours of admission.
We provide mental assessment or evaluation of consumer prior to start of service.
Treatment provided in the home and community to work on barriers and challenges associated with diagnosis.
Assisting consumers and their families with developing community support systems and natural resources to utilize after service has been discontinued.
